Verify Coverage, Benefits, and Network Status
Evernorth® Behavioral Health recommends that you verify a patient’s benefits and eligibility prior to rendering services. To save time and ease your administrative burden, we have created the Coverage Confirmation Grid, which is a fast and simple way to verify benefits and eligibility and find information concerning participating providers, prior authorization requirements, and deductible and out-of-pocket accumulation information.
Coverage Confirmation Grid
You can use the Coverage Confirmation Grid (an Excel spreadsheet) to request the most frequently requested information regarding your patients with Cigna HealthcareSM and/or Evernorth coverage. If you provide employee assistance program (EAP) services, you can also use the grid to request new and/or verify existing EAP authorizations.
Submit a Coverage Confirmation Grid using these simple steps:
Choose the appropriate Coverage Confirmation Grid below and save it to your computer:
Instructions are included in the Grid, but please ensure you complete all fields in each of the applicable spreadsheet tabs. One tab can be used for one and/or up to 100 patients.
- Benefits and Eligibility – Verify benefits and eligibility for your patients and receive deductible and out-of-pocket accumulation information.
- Network Status – Determine if you are contracted with Evernorth Behavioral Health.
- Request New EAP Authorization (on EAP grid only) – Request EAP authorization for patients prior to rendering services.
- Verify Existing EAP Authorization Details (on EAP grid only) – For patients who already have an EAP authorization.
- Save the Coverage Confirmation Grid to your computer as an Excel file.
- Send the completed Excel grid via secure email to EBHCoverageConfirmation@Evernorth.com.
Following your submission
After securely emailing your Coverage Confirmation Grid, you will receive an autoreply email that confirms your grid submission was received.
Completed grids will be returned via secure email with the mint-colored columns updated within one business day from the date it was received.
